If you would like, please click the applicable form to print it out, fill it out at your convenience and bring it with you on your first visit along with a valid form of ID and any insurance cards you may have. What to Expect – Your First Visit
Your initial visit will consist of a consultation, examination and x-rays (if medically necessary). That first visit is all about getting all the information needed to determine if we can help you and if so, how we can best serve you. The initial visit typically lasts about 45-60 minutes. If you have insurance, we will also verify your insurance benefits for you so that we can go over it with you in detail on your second visit. After your first visit, the doctor will combine everything from your health history, consultation, the results of your examination and any diagnostic tests to create a customized plan to help you achieve your health and wellness goals.
What to Expect – Your Second Visit
Your second visit will begin with a report of findings. The doctor will go over with you in detail the findings from your initial visit, your treatment plan recommendations and any financial responsibility you may have for your care. We want you to know up front what the cost to you will be so that any financial concerns may be addressed right away and so you don’t have to worry about any unexpected expenses. The doctor will also take the time to talk with you about any questions or concerns you may have. Your health is important to us and we want to make sure you understand every aspect of your care and how we can best serve you. If you choose to begin care at this time, we will get started right away. When it comes to your health, sooner is always better than later.
